3. Bio (30 words or less)
4. List of organizations that you are currently a continuing education provider for
5. Session titles and short descriptions
6. List of equipment needed for each session and what company provides / manufactures it
7. Three professional references (name, e-mail and phone #)
8. DVD of you presenting at least one of the sessions that you are applying to present
